Good Faith Exam

A Good Faith Exam is required step before your first IV, injection or concierge nursing service at Luxehydra. This quick health screening ensures your safety, is performed by a licensed medical doctor through our telehealth partner Qualiphy, and is valid for (1) one year.

- About the Good Faith Exam 

  • Conducted virtually by a licensed MD via our partner platform Qualiphy

  • Takes only a few minutes to complete.

  • Covers your health history and safety to clear you for treatment.

  • Cost: $27.99 (one-time fee, that covers the Good Faith Exam only, valid for (1) one year).

What Happens Next:

All new clients must complete a short Good Faith Exam by our Telehealth Medical Provider "Qualiphy" before receiving any IV Hydration, Injection, or Concierge Service.

1. Contact Luxehydra.
Option A. - Text 440-290-6267 with your full name, date of birth, phone number, email address, and the service(s) you’re interested in.

OR Option C. - You can click on the purple "EXAM INVITE" button at the top of this page on the left.

2. Receive your secure link.
We’ll personally send your Good Faith Exam link via email during
business hours (Mon–Thursday  7AM - 5 PM,  Friday 7 AM - 8 PM and Saturday 7 AM - 9 PM

3. Complete your Telehealth Exam thru our Medical Provider at Qualiphy
The exam takes about 10 minutes and costs $27.99, paid directly to our Medical Provider Qualiphy (not Luxehydra). Once completed it is valid for one (1) year.

4. Get cleared.
Once approved, you’ll receive confirmation that you’re medically cleared.

5. Book your service.
After you’re medically cleared, return to www.luxehydra.life and click the gold “BOOK NOW” button at the top of this page that will take you over to our scheduling portal to schedule your appointment and complete payment.
